Job Description
Peninsula Outreach Specialist is a full-time, 80-hour-per-payperiod position within the Clinical Access department of the Peninsula Medical Center. This role involves providing care monitoring, in-person support, and engagement with service participants in the community. The specialist will work with consumers in their homes, doctor's offices, or other service agencies, focusing on helping them understand and maintain their health. Peninsula has a history of improving the health of the community and offers comprehensive mental health services.
Requirements
