Job Description
Care Dimensions is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW or LICSW) to join their hospice care team, dedicated exclusively to weekend coverage. The Hospice Social Worker will provide psychosocial assessments and crisis interventions, work closely with the interdisciplinary team, and deliver timely, patient-centered care.
Requirements
- Master's degree in Social Work (MSW) from an accredited institution.
- Current and active LCSW or LICSW license in Massachusetts.
- Minimum of 2 years' experience in hospice, palliative care, or a related clinical setting strongly preferred.
- Demonstrated knowledge of end-of-life issues, bereavement, and grief counseling.
- Exceptional interpersonal, communication, and organizational skills.
- Ability to work independently and manage time effectively during weekend shifts.
- Reliable transportation and valid driver's license (if required for home visits)
