Job Description
CareHarmony is seeking a Remote LPN (Nurse) to deliver value-based care management initiatives to patients. The role involves assessing patients, coordinating resources, providing education, and assisting with medication management. Candidates should thrive in a fast-paced environment and be self-motivated.
Requirements
- Accepting transfers from Patient Enrollment to assess patients.
- Identify and coordinate resources for chronic care management.
- Provide patient education and health literacy.
- Assist with medication management.
- Assist with timely delivery of services (Home Health, DME, etc.).
