Job Description
CareHarmony is seeking a Remote Licensed Practical Nurse (LPN) to deliver value-based care management initiatives for patients. The role focuses on identifying resources, coordinating needs, and providing patient education in a fast-paced environment. This is a full-time, salaried position with a rotational on-call schedule.
Requirements
- Accepting transfers from the Patient Enrollment team to conduct preliminary health assessments.
- Provide patient education and health literacy on chronic conditions.
- Assist with medication management.
- Ensure timely delivery of services to patients.
- Active Compact/Multi-State license (LPN) (LVN).
- Technical aptitude – Microsoft Office Suite
- Excellent written and verbal communication skills
- Remote Requirements (High-speed Wi-Fi, HIPAA-compliant workspace)
- Physical requirements (sedentary, ability to speak clearly & listen attentively, understand/process real-time notes)
- Strong ability to work in a fast-paced environment.
