Job Description
CareHarmony is seeking a Remote Licensed Practical Nurse – LPN (NLC) (LVN) to provide value-based care management initiatives for their patients. This role involves direct patient interaction, coordination of resources, and providing health education. Requirements
- Accepting transfers from Patient Enrollment team to conduct health assessments.
- Resolving patient questions and creating open communication.
- Identifying community resources and coordinating care.
- Providing patient education on chronic conditions.
- Assisting with medication management and ensuring timely delivery of services.
- Active Compact/Multi-State license (LPN) (LVN).
