Job Description
CareHarmony is seeking an experienced Licensed Practical Nurse – LPN (NLC) (LVN) for a full-time, remote position in intake management for their providers. The role focuses on delivering value-based care management initiatives and ensuring timely delivery of services to patients in their network, with a focus on creating an open dialogue and understanding patient needs. This position is suitable for motivated individuals who thrive in fast-paced environments.
Requirements
- Must have an active Compact/Multi-State license (LPN) (LVN)
- Active high-speed Wi-Fi
- Home office or HIPAA-compliant workspace
- Excellent written and verbal communication skills
