Job Description
CareHarmony is seeking a licensed practical nurse (LPN), licensed nurse (LNC), or registered nurse (LVN) to provide value-based care management for providers, focusing on chronic care management patients. The role offers a fast-pace, demanding position with consistent scheduling and opportunities for career growth. The position requires immediate communication and adaptability to a fast-paced, call-heavy environment.
Requirements
- Active Compact/Multi-State license (LPN) (LVN)
- Technical aptitude – Microsoft Office Suite
- Excellent written and verbal communication skills
- Active high-speed Wi-Fi
- Home office or HIPAA-compliant workspace
- Bilingual (preferred)
