Job Description
CareHarmony is seeking an experienced Licensed Practical Nurse (LPN) (NLC) (LVN) to deliver value-based care management initiatives to patients. This remote position involves assessing patients, coordinating care resources, providing health education, assisting with medication management, and ensuring timely service delivery. The company values work-life balance with rotational days off.
Requirements
- Accepting transfers from Patient Enrollment team to assess new patients.
- Provide patient education on managing chronic conditions.
- Assist with medication management and adherence.
- Coordinate with healthcare providers and Home Health services.
- Active Compact/Multi-State license (LPN) (LVN).
