Job Description
CareHarmony is seeking an experienced Licensed Practical Nurse (LPN) (LVN) to deliver value-based care management initiatives for their patients. The role focuses on providing patient education, managing medications, and coordinating care resources. This is a 100% remote position offering a full-time schedule, career growth potential, and a quiet work environment.
Requirements
- Manage patient census with a resolution-driven approach.
- Identify and coordinate community resources.
- Provide patient education on chronic condition management.
- Perform medication management, including reconciliation and refills.
- Assist with home health, DME, and other clinical needs.
- Resolve patient questions and create communication.
- Manage referrals and appointment scheduling.
