Job Description
CareHarmony is seeking a motivated and experienced Licensed Practical Nurse (LPN) (LVN) to deliver value-based care management initiatives for patients. This is a fully remote position with a rotational on-call schedule, offering career growth potential. The role involves patient education, medication management, and coordinating with various healthcare providers.
Requirements
- Manage patient census and close gaps in clinical and non-clinical care.
- Coordinate resources and identify community needs for patients.
- Provide patient education and health literacy on chronic conditions.
