Job Description
CareHarmony is seeking an experienced Licensed Practical Nurse to provide value-based care management for patients. This is a fully remote position with a consistent schedule, career growth potential, and focus on patient education and health literacy. The role involves managing patient census, coordinating resources, and addressing medication needs.
Requirements
- Manage patient census and close gaps in clinical and non-clinical care.
- Identify and coordinate community resources for patients.
- Provide patient education on chronic conditions.
- Perform medication management and ensure timely service delivery.
- Resolve patient questions and manage appointments.
