Job Description
The Licensed Behavioral Health Clinician provides field-based outreach, engagement, and crisis intervention to clients with complex needs living in scattered-site housing. The role involves intervening early during crises to prevent eviction, hospitalization, or disengagement, and serving as a trusted clinical partner within a multidisciplinary housing and care team.
Requirements
- QMHP or QMHP-R
- LPC-A or LPC
- LMFT-A or LMFT
- CSWA or LCSW
- LCSW / LPC HIGHLY Preferred.
- CADC-R, at minimum, preferred.
- Experience providing care coordination, case management, or other services to an under-served adult population experiencing mental illness, substance abuse, and/or houselessness preferred.
