Job Description
The Program Coordinator provides clinical oversight to Specialty Behavioral Health staff and provides direct patient-centered care coordination. Care is provided in the clinic and outside in the community meeting each of our patients “where they are”.
Requirements
- Responsible for overseeing behavioral health programs, ensuring integration with community resources, and maintaining high-quality service delivery.
- Develop and monitor quality improvement plans and program objectives.
- Train, evaluate and provide clinical oversight to Specialty Behavioral Health clinicians that provide direct patient care.
- Serve as liaison for patients, advocates, and community partners; resolve service-related issues.
- Participate in policy development, quality improvement initiatives, and special projects.
- Provide counseling for patients with addiction issues and co-occurring behavioral health disorders while maintaining a caseload.
