CER is seeking a Case Manager to develop treatment relationships with clients with serious and persistent mental illness (SMI/SPMI) in a community-based setting. The role involves driving to client locations, completing documentation, and maintaining a caseload of 8-12 clients. A focus on high quality client care and team support are emphasized.
Requirements
- Bachelor’s degree in Psychology, Social Work, Health, Nursing, Counseling (or similar degree with course work in psychology)
- 6-12 months supervised experience with at least one of the following populations (SMI/SPMI, developmental disabilities, elderly, children, or special needs populations)
- Access to a vehicle and valid driver’s license
