Care Manager Extender

The Care Manager Extender will provide care management functions, documentation, referral and linkage, and monitoring/follow-up. They will work with clients, family, friends, and providers to achieve desired outcomes and provide education for health promotion.

Requirements

• High school diploma or equivalent
• Certified Peer Support Specialists, Community health workers (CHW), or individuals with lived experience with I/DD or a TBI or a behavioral health condition
• 2 years of paid care management type experience with at least 1-year paid experience with population served
