Care Manager Extender

The Care Manager Extender provides care management functions, documentation, referral, and linkage, and monitoring/follow-up under direct and indirect supervision. The role involves referring and linking to needed services, monitoring and follow-up with clients and referrals, and providing education for health promotion.

Requirements

• High school diploma or equivalent
• Certified Peer Support Specialists, Community Health Workers (CHW), Community Navigators, or individuals with lived experience with an I/DD, TBI, or behavioral health condition
• 2 years of paid care management type experience with at least 1 year paid experience with the population served
