Birthing Families Navigator

Requirements

• Provides services including coordination of primary care (prenatal, post-partum, and/or pediatric care) and social services
• Collaborates with fellow Birthing Families Navigators to provide constant floor coverage
• Collaborates with client’s provider and care team
• Displays familiarity and knowledge of the care plan of each client and facilitates referrals and follow-up process
• Completes monthly reports as assigned
