Community Health Worker - Birthing Families Navigation

Erie Family Health Centers is seeking a Community Health Worker to support the Birthing Families Navigation Team. The CHW will maintain client service data, direct referrals, and complete enrollments across various programs. The role requires flexibility with schedule and site, and the ability to work every other Saturday.

Requirements

• High School diploma or equivalent required
• Preferred: Associate degree or HS Diploma or equivalent with 1-2 years of work experience with customer service or data entry
• Community Health Worker Certification a plus
• Experience with data entry preferred
• Experience in healthcare or social services preferred
• Cultural competency to work in a low income, minority environment
• Attention to detail with data and reports
• Interpersonal and oral communication skills
• Intermediate proficiency in Microsoft required
• Bilingual fluency (English/Spanish) required
