We are seeking an experienced Physiotherapist to join our team at Ontario Health atHome, Champlain. The Physiotherapist will assess and determine patient care needs, provide access and referrals to community services, and engage with patients, caregivers, and other healthcare practitioners. The successful candidate will be a key member of our multi-disciplinary team and will have the opportunity to make a meaningful impact in the lives of our patients.

Requirements

• Degree or diploma in Physiotherapy
• Current certificate of registration with the College of Physiotherapists of Ontario
• Related professional experience in a Physiotherapy role
• Current CPR-C certificate
