This position is for an MTM Pharmacist, aiming to enhance medication outcomes in an ambulatory care setting. Requirements
- Current Wisconsin pharmacist license (or eligible)
- Graduate from an accredited college of pharmacy with post-hire curriculum
- Two years of MTM services experience, or one-year post-doctoral residency
- Credentialing with NCQA
- Board Certification in Ambulatory Care Pharmacist (BCACP) or Pharmacotherapy Specialist (BCPS) preferred
- Credentialing following NCQA guidelines
- Previous work providing MTM services as part of a multidisciplinary team
