Care Manager, Behavioral Health

The Behavioral Health Care Manager works as a key member of the collaborative care team within a Primary Care clinic, supporting and coordinating the mental and physical health care of an assigned caseload of primary care patients.

Requirements

• Graduate degree in social work or related field
• Three years of work experience preferably in a healthcare setting providing social work, mental health, and staff supervisory services
• Behavioral health counseling and brief short-term therapy strategies and case management in an ambulatory setting
• Experience with program development and evaluation
• Must have current license as LCSW, PsyD, LPC, LMFT, or CSWA in the State of Oregon
