Job Description
Homeward is a healthcare delivery company dedicated to improving access to care for rural Americans. They partner with communities and health plans to address health disparities, focusing on aligning incentives for clinical outcomes and cost of care. This role is a Care Navigator in Traverse City, Michigan, focused on building relationships with patients and providers to achieve their health goals through proactive outreach and technology utilization.
Requirements
- Completion of a Medical Assistant program from an accredited program or school
- High school diploma or equivalent
- At least two years of experience in high-touch, patient-facing roles, preferably with seniors
- Passion for delivering care in rural America and ability to persuade members to take actions to support their health
- Expertise with the local community, geography, culture, healthcare ecosystem, and available resources
