Job Description
Join our team as a Case Manager at Hospital for Behavioral Medicine in Worcester, MA. The Case Manager will plan and coordinate collateral work for patients, communicate with family members and community supports, and oversee discharge planning and communication with outside parties.
Requirements
- Bachelor's Degree in Social Work or a clinical related mental health field
- Minimum of one year experience in related disciplines with hospital experience strongly preferred
- CPI training within 30 days of employment and prior to assisting in a restraining procedure
