Emergency Department Follow-Up (LPN Required)

Imagine Pediatrics is seeking an Emergency Department Follow-Up (LPN) to assist patients in post-hospital care, providing telephonic, in-person support, and completing necessary follow-up steps. This role requires a dedicated individual passionate about reimaginining pediatric health care and creating a world of supportive care for children with complex needs.

Requirements

• Associates' degree or higher
• 2+ years of relevant clinical experience
• Emergency or inpatient settings preferred
• Experience with medically complex patient populations preferred
• Pediatrics preferred
• Licensed Practical Nurse required
