Emergency Department Follow-Up (LPN Required)

Imagine Pediatrics is a tech-enabled pediatrician-led medical group committed to transforming pediatric care for children with special health needs. This Emergency Department Follow-Up position provides post-hospital support to patients, engaging them with education and coordinating care. It's a remote position requiring a passion for reimagining pediatric health care.

Requirements

• Associates' degree or higher
• 2+ years of relevant clinical work experience
• Emergency or inpatient settings preferred
• Pediatrics preferred
• Licensed Practical Nurse license required
• Knowledge of Medicaid population preferred
• Bilingual Spanish required
