Emergency Department Follow-Up (LPN Required)

Imagine Pediatrics is seeking an Emergency Department Follow-Up provider (LPN) to assist patients in post-hospital care. The role involves proactive engagement, virtual/telephonic visits, medication coordination, and escalating concerns to the clinical team. It's a role focused on enhancing care, not replacing existing teams, emphasizing compassion, creativity, and a commitment to children with medical complexity.

Requirements

• First and foremost, you’re passionate and committed to reimagining pediatric health care.
• Associates' degree or higher
• 2+ years of relevant clinical work experience
• Emergency or inpatient settings preferred
• Experience with medically complex patient populations preferred
• Bilingual Spanish required
