Job Description
This Care Manager role supports long-term services and coordination for members with high-need potential, ensuring successful completion of goals and contributions to an overall care strategy. The role involves comprehensive member assessments, waiver enrollment/disenrollment, care plan development, monitoring, and collaboration with multidisciplinary teams. It requires a blend of clinical, behavioral health, and community resources expertise, focusing on both members and their support network.
Requirements
- At least 2 years health care experience, including 1 year with persons with disabilities/LTSS
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
- Bachelor's degree in a health care related field (required when applicable)
- Valid driver's license, reliable transportation, and adequate auto insurance
