Care Manager, LTSS

This Care Manager, LTSS position supports members with high-need potential, ensuring their progress toward desired outcomes through comprehensive assessments, waiver enrollment, care plan development, and collaboration with multidisciplinary teams. The role focuses on integration of services, motivational interviewing, and preventing barriers to care. The position requires driving for local travel, based on contract or state requirements.

Requirements

• At least 2 years health care experience, including at least 1 year working with persons with disabilities/chronic conditions long-term services and supports (LTSS), and 1 year of experience in care management, or experience in a medical and/or behavioral health setting
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). Clinical licensure and/or certification required only if required by state contract, regulation, or state board licensing mandates.
• Valid driver's license, reliable transportation, and adequate auto insurance.
• Demonstrated knowledge of community resources.
• Ability to work independently and adapt style.
• Excellent time-management and prioritization skills
• Strong communication skills
• Microsoft Office suite/applicable software proficiency
