Care Manager, LTSS

This role provides support for care management and coordination of long-term services for members with high-need potential. The Care Manager will ensure member progress towards goals, foster collaboration between disciplines, and promote continuity of care. They will develop and implement care plans, monitor performance, and advocate for member needs.

Requirements

• Completes comprehensive member assessments within regulated timelines.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans in collaboration with members, caregivers, and healthcare professionals.
• Monitors care plan effectiveness, documents interventions, and suggests changes.
• Promotes integration of services for members including behavioral health and LTSS.
• Assesses for medical necessity and authorizes waiver services.
• Evaluates covered benefits and advises funding sources.
