Job Description
The Case Manager (CM) will be responsible for managing assigned patients, determining appropriate Level of Care, developing care plans, and ensuring timely implementation. This role involves collaboration with various teams to improve patient outcomes, efficiency, and resource utilization. The CM also documents clinical reviews and may provide discharge planning support. The role requires experience in utilization review and discharge planning.
Requirements
- Acknowledge and document variances affecting LOS.
- Conduct follow-up for delayed treatments and reporting.
- Plan/develop specific goals with physician and interdisciplinary team.
- Maintain a working knowledge of utilization review requirements.
- Collaborate with physician and managed care companies on denial appeals.
- Document UR notes in medical record in OMH guidelines.
