Job Description
The Case Manager (CM) will manage case management for assigned patients, ensuring appropriate Level of Care is determined and developing plans of care. Collaboration with interdisciplinary team members is essential. The CM actively participates in initiatives aimed at reducing length of stay (LOS) and improving efficiency. This role involves documentation, collaboration, and coordination of services to support patient and organizational goals.
Requirements
- Review new admissions and determine utilization review, discharge planning, and/or case management needs.
- Collaborate with physicians and other clinicians to expedite diagnostics and treatment.
- Document clinical reviews in Case Management Documentation system.
- Maintain working knowledge of Utilization Review requirements.
