Job Description
Western Connecticut Home Care is seeking Occupational Therapists to join their team, providing services in the comfort of the patient's home. Requirements
- Develops, implements and evaluates care plans.
- Provides occupational therapy services in accordance with the plan of care.
- Maintains records on visits to clients.
- Participates in case management conferences.
