Job Description
Western Connecticut Home Care is seeking Occupational Therapists to join their home healthcare team as part of the Nuvance Health network. They provide home healthcare services with a focus on upper extremity function, independent living, and cognitive perceptual motor function, collaborating with physicians and families for optimal patient care. This is a per diem position with flexible schedules.
Requirements
- Develops, implements, and evaluates individual care plans.
- Provides direct occupational therapy services.
- Maintains accurate records of client visits.
- Participates in case management conferences.
- Provides education to patient and family members.
