Job Description
Western Connecticut Home Care is seeking Occupational Therapists to provide services in the comfort of the patient's home. This role focuses on rehabilitation in upper extremity function, maximizing independence, and cognitive perceptual motor function, working as part of a coordinated home health team. Requirements
- Develops, implements, and evaluates care plans in partnership with patients, representatives, caregivers and physician(s).
- Provides direct occupational therapy services in accordance with a physician's plan.
- Participates in case management and communicates with healthcare practitioners.
- Identifies patient needs and makes referrals.
- Supervises care of COTA(s) and home health aides.
- Re-evaluates and adjusts treatment plans as indicated by physician recommendations.
