Pathways Home Health, Hospice and Private Duty

Pathways Home Health, Hospice and Private Duty is seeking a Part Time Licensed Vocational Nurse (LVN) to provide skilled nursing visits to patients and their families as requested by Intella Triage. The LVN will document observations and interventions related to the patient’s environment, physical, mental, emotional and social status, and communicate care with other members of the Interdisciplinary Group (IDG) or team, including patient’s physician.

Requirements

• California LVN license
• Preferred ‘IV Blood Withdraw’ on discipline license
• Graduate of an NLN accredited nursing school
• Minimum of one-year experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing
• California driver’s license and automobile insurance with minimum Bodily Injury Liability coverage Insurance per agency policy
• Current CPR Certificate– American Heart Association ‘BLS Health Care Provider’ requested upon renewal
