Pathways Home Health, Hospice and Private Duty

Pathways Home Health, Hospice and Private Duty is seeking a Part Time After Hours Triage RN to coordinate hospice and/or home care service delivery after regular business hours. The RN will work from the office or home, providing clinical and administrative direction to field staff, and must be highly motivated, self-directed, and able to work well in a multidisciplinary team setting.

Requirements

• California RN license required
• PHN preferred
• Recent experience in home care or hospice preferred
• Minimum of two years of nursing experience
• California driver's license and automobile insurance with minimum Bodily Injury Liability Insurance per agency policy
• Current CPR certification – American Heart Association “BLS Health Care Provider” required upon renewal
• Excellent nursing assessment, communication, and problem-solving skills
