Pathways Home Health, Hospice and Private Duty seeks a Part Time After Hours Admission/Visit Registered Nurse (RN) to provide skilled nursing visits to patients and their families as requested by Intella Triage. The nurse will assess patient needs, develop individualized plans of care, and coordinate care with other team members.

Requirements

• California RN license
• Graduate of an NLN accredited nursing school
• Minimum of one-year experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing
• California driver's license and automobile insurance with minimum Bodily Injury Liability coverage Insurance per agency policy
• Current CPR Certificate - American Heart Association 'BLS Health Care Provider'
