We recruit, train, and staff local Patient Care Navigators that can serve and in-person or remote liaisons to drive patient care. A technology suite is used to design modern digital pathways for your health system / health plan. Patients or members never have to download an app or set a password; all engagement is delivered through modern native channels (SMS-Text, Email, Phone Call). Patient Care Navigators have access to a Patient Relationship Manager (proprietary software) that seamlessly integrates with any EHR to drive digital journeys, manage task queues, and facilitate follow-up actions such as automated appointment reminders. Our Care Navigation programs have helped health systems / health plans fill in quality-based gaps (i.e., 53% decrease in ER visits for high utilizers), and drive better acquisition, retention, and satisfaction metrics (i.e., 60% conversion of patients with no PCP to an in-network PCP) amongst their populations.
