Job Description
This position provides supportive services to residents at Aeon’s OHP properties, assisting them to maintain housing, access community resources, and foster community engagement and resident leadership. The Resident Connections Coordinator will work as part of a team to enhance the quality of residents' lives, empower them in taking steps to achieve self-sufficiency, and provide flexible services during times that meet the needs of participants.
Requirements
- Provide quality service to participants: welcome new residents, develop supportive professional relationships, and identify referral partnerships with local social service agencies.
- Facilitate resident meetings, implement community engagement activities, and ensure residents have access to express concerns and affect positive change.
- Demonstrate teamwork and collaboration skills with 3 different agency partners, communicate and coordinate services, and complete accurate and thorough documentation and recordkeeping.
- Maintain individual electronic Client Track files, including timely detailed case notes, signed data privacy, intake and discharge summaries, and Releases of Information.
