Family Support Worker - 64003825

This position involves assisting clients to identify and obtain available benefits and social and community services. The Family Support Worker will provide information on agency and community services, conduct home visits to provide assessment of needs, and provide psychosocial assessments and referrals to available resources. The successful candidate will work under the general supervision of the Community Health Nursing Supervisor and will be required to complete a background screening and/or drug screening and participation in direct deposit.

Requirements

• High School Diploma
• Valid Driver's License
• Spanish-English bilingual
• Average computer skills
• Ability to successfully multitask
• Ability to set priorities
• Strong communication skills
• Ability to communicate fluently with Hispanic clients
• Strong organizations skills
• Ability to work well with others
