Job Description
The New York Foundling supports children and families through various programs, including fostering, mental health services, and parenting support. The Mother/Child Residence case planner provides case planning, case management, and social work services to pregnant and parenting adolescents in a maternity residential facility. They collaborate with program staff, ACS, and healthcare providers to develop and implement effective interventions for a safe and supportive environment.
Requirements
- Provide case planning/case management and social work services to pregnant and parenting adolescents.
- Collaborate with clinicians on youth's initial mental health screenings.
- Complete intake interviews and case plans.
- Provide weekly counseling sessions and bi-weekly group therapy.
- Complete FASP and Court Reports/Permanency Hearing Reports.
- Attend all required Court Hearings.
