Clinical Liaison (Home Health)

Westminster Canterbury at Home is seeking a Clinical Liaison to facilitate the admission of hospice or home health potential clients. The Clinical Liaison will educate patients and their families on eligibility requirements and benefits, and provide in-service education sessions and public speaking on hospice, home health, and related topics.

Requirements

• RN or LPN license required
• Minimum of 5 years home health or hospice experience required
• Previous Clinical Liaison experience is a plus
• Excellent Communication Skills
• Valid Drivers License
