Clinical Liaison & Outreach

The Clinical Liaison & Outreach role is responsible for identifying, assessing, and facilitating the admission of eligible patients into the Hospice House, particularly for general inpatient services (GIP) when appropriate and respite care. The role provides education to patients, families, and healthcare professionals about hospice philosophy, services, and benefits, ensuring a seamless transition of care and promoting the organization's mission of compassionate end-of-life care and family support.

Requirements

• Active, unencumbered RN license in the state of practice.
• Minimum 2–3 years of clinical experience (hospice, palliative, home health, or acute care preferred).
• Strong clinical assessment and communication abilities
• Knowledge of hospice philosophy and eligibility criteria
• Proficiency with electronic medical records (EMRs) and Microsoft Office
• Valid driver’s license and reliable transportation
