RN Case Manager (Home Health)

Westminster Canterbury At Home is seeking a RN Case Manager (Home Health) to provide ongoing consultation and education for patients/family members regarding terminal diagnoses. The RN Case Manager will collaborate with the hospice interdisciplinary team and model core values of respect, passion, and professionalism.

Requirements

• One year of nursing experience
• Unencumbered Virginia (or Compact) License as a Registered Nurse
• Current CPR certification (Basic Life Support)
• Experience with Microsoft Office and Google Suite
• Valid state driver’s license
• Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers
• Proof of motor vehicle insurance
• Flu-Vaccinated
