Job Description
The Case Manager is responsible for providing case management and housing stabilization services to homeless Veteran families. The position involves coordinating client intake, providing outreach services, and linking clients to supportive services.
Requirements
- Bachelor's degree in Social Services or related field
- 2 years' experience providing case management services
- Prior experience working with Veterans, homeless and low-income individuals strongly preferred
- Commitment to serving homeless individuals and Veterans
- General knowledge of local community resources
- Proficiency in Microsoft Office, email, internet, and calendaring applications
- Experience with HMIS data entry strongly preferred
- Valid California driver license
- Reliable personal vehicle
- Current personal auto insurance
- MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies
- Ability to receive and maintain criminal records clearance
