Job Description
Case Manager responsible for providing case management and housing stabilization services to homeless Veteran families, conducting initial assessments, providing street and venue-based outreach, and coordinating services to ensure housing stability.
Requirements
- High school degree
- 1 year experience providing direct support services to individuals experiencing homelessness, mental health, substance abuse or other social issues
- Proficient in use of computer and Microsoft office suite (Word, Excel, Outlook and TEAMS)
- Commitment to serving individuals experiencing homelessness
- Ability to interact in a supportive and professional manner with staff and clients of diverse cultural and economic backgrounds
