Job Description
The Housing Case Manager will provide outreach and engagement to people who are unhoused, conduct home visits, assist clients with basic needs, and complete paperwork for rapid re-housing subsidies. The ideal candidate will have experience providing direct support services to individuals experiencing homelessness, mental health, substance abuse, or other social issues.
Requirements
- High school diploma or equivalent
- 1 year experience providing direct support services to individuals experiencing homelessness, mental health, substance abuse, or other social issues
- Ability to complete CPR/First Aid Training
- Experience with Rapid Re-Housing
- Experience with housing retention programs (preferred)
- Lived experience (strongly preferred)
- Proficient in use of computer and Microsoft office suite (Word, Excel, Outlook, TEAMS)
- Experience with Homeless Management Information System (HMIS) (plus)
