Job Description
Multnomah County Health Department is seeking a Team Anchor Provider to provide care to a panel of patients as part of an interdisciplinary team in collaboration with the Primary Care Provider. This position will ensure care coordination and care continuity to patients in primary care settings.
Requirements
- Masters degree or equivalent in nursing from an accredited college or university
- Possession of a valid Nurse Practitioner license OR Possession of a valid Medical Doctor license
- CPR/BLS certification
- DEA certification
