Registered Nurse - Inter Professional Care Team

We are seeking a 1 FTE Registered Nurse to provide clinical services to clients who face barriers to accessing care. The position involves providing care in social housing units and other designated areas, collaborating with external partners, and utilizing professional assessment skills to identify physical, mental, and psychosocial needs of clients.

Requirements

• BScN degree with current CNO registration
• 2-3 years of nursing experience
• Ability to work flexible hours, including evenings and weekends
• Ability to travel for work
• Strong organizational, communication, and leadership skills
• Awareness and sensitivity to socio-economic and cultural issues
• Knowledge and proficiency in current evidence-based practices of primary care delivery
• Proficiency in electronic charting
• Valid driver's license, access to a vehicle, and ability to travel to other service locations
